ATTENTION PARENT:

The Washington Parish School Board is enforcing the current student check-out procedure which states:  "In order for a student to check out of school, the parent must come to school to sign his/her child out.  Phone calls are not acceptable."

However, for cases of emergency only, you may give someone else permission to check your child out of school.  In order to do this, you must collect the original signatures of persons authorized to check your child out of school on the Validated Check-Out Form below.  This form must be completed in ink in the handwriting of the authorized person.  This form will be kept on file at the school office for signature comparisons when students are checked out.

When Validated Check-Out Form is completed, you, the parent, must bring the form to the school and sign it and date it in the presence of the principal or principal's designee.
